Annotatsiya

Kombinatsiyalashgan o‘tkir bosh miya jarohati travmatik omilning og‘irlik darajasiga qarab miya tuzilmalarining bevosita shikastlanishiga hamda uzoq joylashgan organlar faoliyatining buzilishiga olib keladi. Kombinatsiyalashgan o‘tkir bosh miya jarohatlarida neyroendokrin buzilishlar ko‘pincha yetarlicha aniqlanmaydi, bu esa kasallikning ham o‘tkir, ham surunkali bosqichlarida turli asoratlarning rivojlanishiga sabab bo‘ladi. Ushbu tadqiqotning maqsadi bemorlarni boshqarish protokollarini va umumiy yashab qolish ko‘rsatkichlarini yaxshilash uchun kombinatsiyalashgan o‘tkir bosh miya jarohatlaridagi neyroendokrinologik ko‘rsatkichlar bo‘yicha so‘nggi ilmiy dalillarni tahlil qilishdan iborat bo‘ldi. So‘nggi besh yil davomida ochiq ma’lumotlar bazalarida chop etilgan ilmiy maqolalar tahlil qilindi. Tadqiqot natijalariga ko‘ra, bosh miya jarohatlaridan keyin eng ko‘p uchraydigan neyroendokrin holatlar gipofizar yetishmovchilik, qandsiz diabet, gipotalamus disfunksiyasi va kognitiv buzilishlar ekanligi aniqlandi. Past arterial bosim (ham tizimli, ham intrakranial), anemiya va gipoksiya markaziy endokrin bezlarda ishemik o‘zgarishlarni yuzaga keltirishi mumkin. Kombinatsiyalashgan travmatik bosh miya jarohatlarining o‘tkir bosqichida somatotrop, gonadotrop va adrenokortikotrop gormonlar yetishmovchiligi belgilari ustunlik qiladi. O‘tkir bosqichdagi vazopressin yetishmovchiligi miya shishi rivojlanish xavfining ortishi bilan, oksitotsin yetishmovchiligi esa neyrodegenerativ patologiyalar rivojlanishi bilan bog‘liq. Neyroendokrin buzilishlarning differensial diagnostikasi anamnestik ma’lumotlar hamda bemorning biokimyoviy va gormonal ko‘rsatkichlari asosida amalga oshirilishi kerak. Olingan ma’lumotlar travmatik bosh miya jarohati bilan og‘rigan bemorlarni boshqarishda nevrologik buzilishlar bilan bir qatorda neyroendokrin buzilishlarni o‘z vaqtida aniqlash muhimligini ta’kidlaydi va ular birlamchi diagnostika hamda reabilitatsiya bosqichlarida qo‘llanishi mumkin.
